Agent Contracting – must be submitted 7-10 days prior to submitting your first application

  • Complete and submit contracting kit found on our website bordenhamman.com
  • If contracting has been previously completed, email the following to licensing@bordenhamman.com
    • a request for “pre-appointment to OMAHA INSURANCE COMPANY for MedSupp”
    • Include the resident state of proposed insured and the state the application will be signed if different

 

Regarding TRANSFERS of existing contracting:

  1. Required transfer paperwork can be requested from: licensing@bordenhamman.com
  2. Date on new business applications must be AFTER your contract transfer has been processed

 

NOTE: Omaha Ins. Co. will require you to complete a brief online MedSupp training

Instructions for this training will be sent from Omaha Ins. Co after your contracting is processed.

New Business Application  – can be faxed or mailed:

  • Current forms and rates can be pulled at any time from the BORDEN HAMMAN website on the Print Applications page
  • Your Commission Code is M8this is requested in a few spaces on the applications

(upper right corner of the first page). It is not meant to be your agent number.

 

  • If paying the initial premium via bank draft:
    • The enclosed coversheet (required)
    • Completed application  (as directed on the application coversheet)
    • MUST include the ACH form to pay for the INITIAL premium

  • If paying the initial premium via check
    • Initial premium MUST be submitted with application (either by check or ACH form)
    • Completed application  (as directed on the application coversheet)

  • Then, once you are contracted, try the new e-App
    • You can now complete and submit applications online through iGo.
